Farmington Community Library Equipment Problem Reporting

The Equipment Problem Reporting Process consists of the following steps:

1. Click on the link below to open a fill-in .pdf Equipment Problem Report Form (EPR).
2. Type in pertinent information in each yellow field; move from field to field using the TAB key. Radio buttons are selected by clicking them with the mouse.
3. When you have completed your portion of the Form, print it out and place it in Diane Serresseque's Mail Box. Eventually, we will upgrade our version of Acrobat Professional to enable emailing the form instead.
