Use this site to file a complaint about Medicaid fraud, or a patient abuse claim. You can review the information which identifies the different kinds of fraud or patient abuse/neglect investigated by the Attorney General's Health Care Fraud Division. If you suspect either Medicaid provider fraud or resident abuse/neglect in a Medicaid/Medicare-funded facility, you may fill out the appropriate complaint form, fraud or patient abuse, (and don't forget to print a copy of the form before submitting it ) and electronically submit the form to the Health Care Fraud Division.

Pursuant to the Michigan Public Health Code, P.A. 368 of 1978, as amended, The Michigan Department of Community Health is required to publish the names of disciplined individuals.
To comply with this requirement, the Bureau of Health Professions frequently publishes Disciplinary Action Reports, available in .pdf on this site. A Disciplinary Action Report (DAR) lists the disciplinary actions taken against health professionals who are licensed and regulated by the various health boards within the Bureau of Health Professions. The report also includes updated information regarding licensees who have appealed the Board's action to a higher court. The report includes the names of the health professional, their professional license number, the type of disciplinary action taken, the effective date of the action and the general nature of the complaint.

As most consumers know, the prices of prescription drugs can vary from pharmacy to pharmacy. The Michigan Department of Community Health (MDCH) has created this website to help the citizens of Michigan compare prices among pharmacies, since drug prices typically are not advertised or even displayed.
The drugs selected for this website are 150 of the most commonly prescribed drugs in Michigan as reported by MDCH's Medical Services Administration; prices are obtained from Medicaid prescriptions and represent many of the State's retail pharmacies. This web site allows citizens to search for information on particular listed drugs in a geographic region by pharmacy.
Since some of the prices listed on this website could be up to 6 months old, consumers should call ahead to their pharmacy of choice to check the latest prescription drug price - before traveling to purchase a prescription drug.

Michigan Medicare/Medicaid Assistance Program
Michigan Medicare/Medicaid Assistance Program (MMAP) is a free health-benefit counseling service. Since 1984, MMAP has provided education and counseling assistance to Michigan’s Medicare and Medicaid beneficiaries and their families or representatives.
MMAP accomplishes its mission through 48 MMAP sites housed in regional Area Agencies on Aging, County Departments on Aging, Senior Services Agencies, Commissions on Aging and other similar organizations located throughout Michigan.
Currently, MMAP has over 500 highly trained and certified volunteers and volunteer coordinators. These dedicated and compassionate people, many of them seniors, answer questions and act as guides through the Medicare and Medicaid programs.
In 2003, MMAP assisted over 22,000 Michigan beneficiaries and saved them $17 million in out-of-pocket expenses, bringing peace of mind to those who are often confused and frightened when they deal with unfamiliar and difficult government systems.
